What triggers it

The onboard charger detects the accumulated difference between estimated current and measured current is too large / outside acceptable range.

When it clears

The onboard charger will restart the affected converter(s) automatically until the maximum number of retry attempts has been reached. A charge cable cycle (disconnect and then reconnect to the vehicle) may also be required, after which the onboard charger must detect the accumulated difference between estimated current and measured current is again within acceptable range.
